
Case Management Part II: Seamless Service Delivery and Resource Networks is a 1.5-hour practitioner-centered course that builds on the foundational case management process introduced in Part I. Participants expand their understanding of limited, moderate, and concentrated case management; apply strategies for seamless service provision; strengthen barrier-removal and follow-through practices; and build viable resource and partner networks that support client outcomes.

Case Management Part I is a 1.5-hour workshop that addresses the case management process within the career development environment. Participants are provided with an overview of the case management process including components, stages, and types as they pertain to various career development positions.

Case Management Part II builds upon the foundational concepts introduced in Case Management Part I. This course explores advanced strategies for creating seamless service provision models, addressing client barriers, organizing service delivery, and building viable networks of community resources and professional relationships.
Participants will examine practical case management situations and identify strategies that promote coordinated, accessible, and client-centered services.
